Comorbidity in borderline personality disorder and adult attention deficit hyperactivity disorder in the context of impulsivity and emotional dysregulation

Introduction
In a significant proportion of people diagnosed with Borderline Personality Disorder (BPD) and adult Attention Deficit Hyperactivity Disorder (aADHD) comorbid mental disorders, such as mood, anxiety, personality and substance use disorders can be detected. BPD and aADHD present with a partial overlap in the clinical symptoms, including increased impulsivity levels and difficulty in emotional regulation. Higher impulsivity and emotional dysregulation (ED) can result in impaired global functioning or damaged social relationships.
ObjectivesThe aim of this study was to assess the comorbid psychiatric diagnoses of the two patient groups, and explore the possible role of ED and impulsivity in the background of the different comorbid disorders.
MethodsData about BPD (N=49) and aADHD (N=60) patients were analyzed based on the M.I.N.I. Plus 5.0 and SCID-5-PD structured clinical interviews. Participants were further investigated with online questionnaires: e.g. Barratt Impulsiveness Scale (BIS-11) Difficulties in Emotion Regulation Scale (DERS). For measuring the influence of impulsivity and ED in the development of comorbid disorders binary logistic regression was used.
ResultsOur results showed comorbidity rates similar to previous findings in BPD patients, but lower rates were observed in aADHD. Elevated levels of ED increases the risk of suicidal ideation, mood, anxiety and eating disorders. Based on our data increased impulsivity can reduce the chance of comorbid anxiety disorders.
ConclusionsThe results provide insight into the pattern of comorbid disorders, role of ED and impulsivity in people diagnosed with aADHD and BPD in Hungary. Understanding their underlying mechanisms helps to establish an accurate diagnosis, which affects treatment effectiveness.

Introduction
High levels of impulsive behavior represent a core symptom of different psychiatric conditions, such as Attention Deficit Hyperactivity Disorder (ADHD), Borderline Personality Disorder (BPD), impulse control and conduct disorders, bulimia nervosa, substance use disorders, and other maladaptive behaviors. Impulsivity is a multidimensional construct, having at least three factors.
ObjectivesOur aim was to describe the impulsivity profile in adult ADHD and BPD patients in comparison with a healthy control group, taking into consideration the different impulsivity factors.
MethodsaADHD (n=80) and BPD Patients (n=60) were recruited, based on the DSM-5 criteria. Control subjects (n=80) were screened using Derogatis Symptom Checklist (SCL-90). Comorbidities were assessed by structured clinical interviews. Participants were further investigated with online questionnaires including the Barratt Impulsiveness Scale (BIS-11), Difficulties in Emotion Regulation Scale (DERS), and neuropsychological tests, like Rogers’ decision-making test.
ResultsBased ont the BIS-11 and DERS results, significantly higher levels of impulsivity (motor, attentional, non-planning) and difficulties in emotion regulation were present both in the aADHD and BPD groups, compared to the control group. Impulsivity factors were more characteristic to aADHD, emotion dysregulation was more specific to BPD. In the Rogers test, aADHD group was significantly slower in decision-making, while in BPD decision quality and risk-taking were affected.
ConclusionsImpulsivity profiles of the two disorders are different, which leads to the assumption of potentially altered pathway of developing impulsive behavior. As a neuropsychiatric condition, impulsivity in aADHD is related to neurobiological dysinhibition, in BPD impulsive behavior is attached to emotionally involving situations, and emotional dysregulation rooted in childhood adverse events.

Exposure to childhood maltreatment (CM) may disrupt typical development of neural systems underlying impulse control and emotion regulation. Yet resilient outcomes are observed in some individuals exposed to CM. Individual differences in adult functioning may result from variation in inhibitory control in the context of emotional distractions, underpinned by cognitive–affective brain circuits. Thirty-eight healthy adults with a history of substantiated CM and 34 nonmaltreated adults from the same longitudinal sample performed a Go/No-Go task in which task-relevant stimuli (letters) were presented at the center of task-irrelevant, negative, or neutral images, while undergoing functional magnetic resonance imaging. The comparison group, but not the maltreated group, made increased inhibitory control errors in the context of negative, but not neutral, distractor images. In addition, the comparison group had greater right inferior frontal gyrus and bilateral frontal pole activation during inhibitory control blocks with negative compared to neutral background images relative to the CM group. Across the full sample, greater adaptive functioning in everyday contexts was associated with superior inhibitory control and greater right frontal pole activation. Results suggest that resilience following early adversity is associated with enhanced attention and behavioral regulation in the context of task-irrelevant negative emotional stimuli in a laboratory setting.

The recently published diagnotic criteria of Alzheimer's disease (AD) includes biological markers (determination of b-amyloid 1-42 (Ab), tau and phospho-tau) from cerebrospinal fluid (CSF), but their positive predictive value proved low, therefore a new biomarker indentification is needed. Apoptosis plays crucial role in AD pathomechanism. Ab interacts with mitochondrial proteins causing oxidative stress. That induces the impairment of energy metabolism promoting tau phosphorylation indicating apoptosis.
The purpose of this study was to identify pathognostic and apoptotic proteins in the CSF of AD patients.
Pooled CSF samples of 5x5 AD patients and 5x5 controls were used. ELISAs were performed to determine the concentrations of Ab, tau and phospho-tau. 10 ultrasensitive slides of peptide microarrays with 653 antigens were used. If the ratio of technical repeats of AD and control samples was under 0.65 that meant decreased protein concentration.
Apolipoprotein D (0,62), DNA polymerase gamma (0,51), parkin (0,58) and methylated-DNA-protein-cysteine methyltransferase (0,56) showed decreased concentration in AD in four chip pairs. The concentration of cyclin-dependent kinase 5 (0,45), granzyme B (0,37) and prostate apoptosis response 4 proteins (0,60) reduced in three chip pairs.
These data contribute to the database that will be used to elucidate the new biological markers for AD. Our findings may have implications for both the diagnosis and the understanding of AD pathogenesis, by defining a patient-specific signature to diagnose AD. Using these 7 biomarkers to identify AD prior to the onset of clinical symptoms and associated with neuronal loss enables novel clinical trial design and early mechanismbased therapeutic intervention.

We have completed a kinematical analysis of 184 selected Planck Galactic Cold Clumps (PGCC) (Planck Collaboration, 2015) in order to understand better the stability of molecular clouds and the induced star formations. Most of our clumps are in the second quadrant of the Galaxy. For the investigation we used 12CO, 13CO, C18O line observations which covered the 184 PGCC fully or partially. The majority of the data were observed with the Osaka 1.85 m telescope in Japan (Nishimura et al. 2015) and we also have some observations with the KOSMA 3 m telescope, from Switzerland (Kramer et al. 2000) and with the IRAM 30 m telescope, Spain (www.iram-institute.org).

The risk for many chronic diseases appears to be mediated in part by birth weight. Among Aboriginal Canadians, the prevalence of end-stage renal disease and cardiovascular disease risk is disproportionately high, largely because of elevated diabetes prevalence. The relationships between birth weight (and other potential risk factors) and diabetes, hypertension, proteinuria and overweight/obesity were explored in 1439 rural Albertans (Canada), of whom 67.3% were Aboriginal. At voluntary outreach screening programs, demographic and clinical data were measured and recalled birth weights recorded. Statistical modeling using logistic regression was used to evaluate the relationships. In the final adjusted models, associations remained for low birth weight and proteinuria [odds ratio (OR) 2.36; 95% CI 1.24–4.49], as well as for high birth weight and overweight/obesity (OR 1.58; 95% CI 1.00–2.53). These findings emphasize the need to strive for healthy pregnancies, with appropriate weight gains in these and other disadvantaged populations around the world.

Summary
The role of body weight on human reproductive function and fertility has received considerable attention in the medical literature. Time-to-pregnancy studies are very useful in determining the overall effects of any exposure on overall fertility. Studying the role of overweight and obesity among couples undergoing assisted reproductive technology (ART) can serve multiple purposes. First, it provides clinically useful information addressing the specific needs of infertile couples. Second, ART provides a unique opportunity to study the role of obesity on reproductive physiology and pathology. Obesity leads to a wide range of systemic alterations including changes in circulating levels of adipokines, reproductive hormones, markers of endothelial dysfunction, and systemic inflammation, as well as metabolic disturbances in lipoprotein metabolism, glycemic control, and increased insulin resistance. The existing evidence on whether clinical management of obesity has any effects on fertility is mostly limited to women with polycystic ovary syndrome (PCOS).

The defects in InP epitaxial structures (containing in part Pt nanoparticles) and SiGe/Si heterostructures, and their electrical behaviour are studied. In InP structures pinholes, in SiGe/Si structures misfit dislocations were observed in the epitaxial layers. These defects yielded different anomalies of the electrical behaviour, as excess and leakage currents, instabilities, anomalous temperature dependence and anomalous apparent barrier height. It is shown that inspite of anomalies, the electrical measurements provide useful and reliable information about the structures.

Self-assembled β-FeSi2 quantum (QD) dots were grown on n-type Si and investigated in this work. Secondary electron images show the shape and distribution of the quantum dots depends on the temperature and thickness of the Fe deposition. Electrical characteristics were measured in MIS devices prepared by covering the quantum dots by an oxide or by a photoresist layer. The cold deposited and subsequently annealed Fe layers were found to generate large concentration of deep level defects, compensating the upper few microns layer of the silicon wafer. Reactive deposition epitaxy (RDE) growth of QDs – where the iron is deposited on hot substrate—generated much lower concentration of defect, in some devices with characteristics comparable to the reference wafer. The significant scatter on the surface is attributed to inhomogeneous growth conditions and to residual surface contamination on the surface.

Lateral conductivity effects have been investigated in self-organised InAs quantum dot (QD) structures grown in a GaAs matrix with different cap layers. Current-voltage (I-V), capacitance-voltage (C-V), DLTS, capacitance and conductance frequency dependence, fast defect transient (FDT), and electron beam induced conductivity (EBIC) measurements were applied. The conductivity in the QD plane decays within a distance of 10 microns. The capacitance transients are dominated by the local QD-plane transversal conductivity and by the free carrier transport in the cap layer. The nonequilibrium free carrier created by electron beam excitation develop a potential barrier at macroscopic distances from the electrical contacts.

Highly oriented 2212 BSCCO thick films were prepared on polycrystalline MgO substrates using a melt/crystallization technique. Results compare very favorably with those on single crystal materials.1,2 The melting, quenching, and annealing processes were all found to be important in the development of a good microstructure and good superconducting properties. The best results (Jc ∼ 2000 A/cm2 at 64 K and 6000 A/cm2 at 4.2 K) were obtained on films which had been melted, quenched to room temperature, heated to 860°C where they were annealed before slow cooling to room temperature in oxygen, lightly polished, and reannealed at 860°C.

The shape of the normalized temperature derivative of the dc susceptibility provides useful information about superconducting materials. We propose that the position of the derivative peak be defined as the transition temperature Tc and the width of this peak define its sharpness.
